How Is a Care Plan Used?

The lead care manager and multidisciplinary care team work with the member to implement their care plan. The care plan guides the care they receive to help them improve their health outcomes and achieve their health goals. Services may be provided in person or through other communication methods that work for the member. The care plan is reviewed together with the member and revised over time, based on the member’s progress and changes in their needs.
